The Process

To become our customer please follow these easy steps:

Step 1.  Print the:
Prospective Customer Survey &
Tax Exemption Certification Forms.  If you wish to apply for credit terms, also print the: Credit Application form

Step 2.  Fill out the forms with your information. (Note: these are pdf files and may be filled in online before printing).

Step 3.  Sign and return to Cinderella Inc. by fax or US Mail. Forms MUST be complete AND signed.
